The Jamestown Cup 2001 - Medical release form

Medical Release Form

No player may play without a valid medical release form.

Medical Release forms may be from your club! Note: use of this form is not mandatory but may be used if team/club specific forms are not available.

There are several ways that you can obtain a medical release form for the tournament...

1. Call the VLSC soccer office @ 757 253-1947and request that one be mailed/faxed to you.

2. Write to "The Jamestown Cup" • 5700 Warhill Trail • Williamsburg, VA 23188 and request that one be mailed to you.

3. Print out a PDF version of the medical release form right from this page. You will need to have Adobe Acrobat installed to do this.
