No player may play without a valid medical release form.  

Medical Release forms may be from your club!  Note: use of this form is not mandatory but may be used if team/club specific forms are not available.

Medical Release Forms do not have to be notarized!

There are several ways that you can obtain a medical release form for the tournament...

1. Call the soccer office @ 757 253-1947 and request that one be mailed/faxed to you.

2. Write to The Williamsburg Cup • 5700 Warhill Trail• Williamsburg, VA 23188 and request that one be mailed to you.

3. Print out a PDF version of the medical release form right from this page. You will need to have Adobe Acrobat installed to do this. If you do not have Acrobat installed, bookmark this page, click on the link below to get it, follow the directions, and then return to this page and print out your application.
